
Management of placental
abruption
●
The management of abruption should be individualized on a
case-by-case basis depending on the severity of the abruption
and the gestational age at which it occurs
●
In cases where fetal demise has occurred, if there is no
indication for CS,vaginal delivery is preferable. When abruption
occurs at or near term FH is positive and maternal and fetal
status are reassuring (mild abruption), conservative
management with the goal of vaginal delivery may be
reasonable. Most of the perinatal morbidity and mortality is
associated with
or maternal compromise, delivery may often be
delayed until fetal maturity is attained
●
in the presence of fetal or maternal compromise
(massive abruption) , prompt delivery by cesarean is often
indicated.
