Functioning
FOOT ULCER SCENARIO
-Ulcer in left foot irregular in shape , 3-4 diameter , 1 cm in
depth ,
Margin is outer blue , inner red ) , Sloping edge , floor
contain healthy granulation tissue rd in color without
slough or discharge , the surrounding skin is blue
HERNIA SCENARIO
-Standing
.Unilateral inguinal swelling on the right side ,globular ,
6cm in width ,4 , smooth surface not nodular reach to
scrotum
.The skin contains no scar no sinus, no redness no dilated
veins and discharge and visible pulsation no visible peristalsis
.presence of two testes
.-ve visible cough impulse
Palpation
.warm not tender
. Soft and elastic , well-defined reg edge slipping ,not
compressible not reducible not pulsatile no thrill , no
fluctuation
not fixed to underlying tissue not fixed to skin no tethering
.-ve cough impulse (visible ,palpable )

THYROID SCENARIO
Inspection
.Unilateral neck swelling on the right side , irregular , 6-4 ,
borders Smooth surface not nodular
.The skin contains no scar no sinus, no redness no dilated
veins and discharge and visible pulsation ,
.the swelling moves with deglutition without upward movement
on protrusion of tongue
Palpation
-front : not tender , warm , trachea
-back :
. firm consistency , well-defined reg edge not slipping ,not
compressible not reducible not pulsatile no thrill , no
fluctuation
.not fixed to underlying tissue not fixed to skin , no tethering
-move with swallowing
.carotid pulse(berry sign ) and lymph node
WOUND SCENARIO
-Inspection
Upper Midline abdominal incision , regular shape , 10cm
Red margin , no redness or bluish discolor surrounding tissue
, no discharge
Suture is interrupted ,of non-absorbable monofilament silk
material
.may i ask him to do cough , to see if there is hernia ?
-palpation
Warm , tender , without crepitation
.the wound is clean not dirty or infected
