Glomerulus with thickened pink capillary loops in a patient with lupus nephritis

The periarteriolar fibrosis ("onion skinning") seen in the spleen in patients with SLE

Here is a glomerulus in which the capillary loops are markedly pink and thickened such that capillary lumens are hard to see. This is lupus nephritis.

EM; Amyloid

"apple-green" birefringence In polarized light + Congo red

Kidney Amyloid deposited primarily in the glomeruli, lead to thickening of the mesengial matrix with widening of the basement membrane, it also deposited in the peritubular tissue, arteries & arterioles

Spleen splenic follicles …….Sago spleen. red pulp …………….Lardaceous spleen

Diagnosis Site of biopsy:kidney, gingival, rectum, skin, or omental fat Stains:H&E, Congo red, Iodine, … Confirmation can be obtained by Electron microscopy, non-branching fibrils of indefinite length and a diameter of approximately 7.5 - 10 nm.
