Over Denture overlay denture super imposed denture

Definition
Partial or complete denture that covers and rest on one or more remaining teeth or root or implant.

Advantages

1.conservative. 2.Decrease resorption. 3.Natural feeling (properioception). 4.Enhace retention and stability. 5.Sychological support. 6.Simple technique. 7.Esthetic measurement be high. 8.Periodic visit to maintain good oral hygiene.

Disadvantages

1.Caries periodontal problem. 2.Expensive. 3.Need more visit. 4.Patient must be educated and cooperative. 5.Over denture is more bulky than conventional denture. 6.Greater maintenance is required.

Indication

1.Patient selection (healthy and cooperative). 2.Poor prognosis for conventional denture. a. V-shape palate. b. Knife edge ridge. C. xerostomia. d. Large tonque. e. Muscle paralysis. f. Severely elderly patient who have poor experience to conventional denture. 3.Severley tilted remaining teeth. Unsuitable for conventional denture

4.Over denture indicated for teeth with grade one and two mobility, because I act as splint. 5.When the prognosis of the remaining teeth is very poor over denture can serve as transitional denture prior to loss of all teeth. 6.Number and position of the remaining teeth. 7.Prognosis of the remaining teeth (Bone, Crown/root ratio, root form and number)

Contraindication

When another method might give superior results fixed or removable prosthesis. Psychologically some patients can not accept any type of removable denture. when the patient can not maintain abutment teeth and periodontal health. When systemic complication make the clinical procedure un satisfactory like diabetic. Bony under cut can not be corrected surgically.

Type of tooth preparation

1.Simple tooth preparation without copying. 2.Tooth preparation with cast copying. 3.Endodentic therapy and amalgam filling. 4.Endodentic therapy with cast copying. This by using : a. Stud attachment. b. Bar attachment

Sequence of treatment

1.Examination. 2.Treatmen planning. 3.Preparatory treatment. a. Relief pain. b. Periodontal treatment and reestablishment of good oral hygiene. c. Conservative treatment (amalgam. Composite). d. Over denture construction. e. Maintenance and follow up.

Examination visual and digital of abutment (caries, periodontal condition , rotation , presence of under cut.

Record

1.Cast a. Inter ridge space. b. Occlusion of opposing teeth. 2.Radiograph. 1.Bone support. 2.Crown root ratio. 3.radiolusency.

No and Locations of teeth

1.Ideal condition four abutment 2 anterior and 2 posterior. 2.Three abutment 1 anterior and 2 posterior. 3. Two abutment usually two canine. 4.One abutment available it can be used for support.

denture insertion

1.Instruction and home care for abutment. 2.Use pressure indicating past to check contact between convex surface of abutment and concave bearing surface of denture. 3.Perfect occlusion by selective girding. 4.Place patient in recall system see patient once monthly for first six month then recovery 3 months

Treatment of reduced vertical dimension

1.Teeth attrition due to bruxism. 2.Presence of pathological problems. Procedure A temporary over denture is used for reestablishment of the corrected occlusal vertical dimension. gradually increase and make crowns over the posterior teeth. After adaptation for new vertical dimension the increase in the vertical dimension is gradually by increasing it 0.5 mm until reach the final corrected one.


Post insertion care
1.Recall. 2.Home Care. 3.Temporary or permanent relining. Purpose o Recall Visit: 1.Check if there is any inflammation. 2.Check if there is any plaque accumulation around the copying. 3. Check if there is any secondary or root caries. 4. Check if there is any resorption.
