Acrylic partial denture

Acrylic partial denture
Definition: Is a dental prosthesis that substitute teeth and associated structures in partially edentulous arch made from acrylic resin and can be removed and replaced at will.

Interim Removable Partial Dentures

Temporary RPD: a removable prosthesis that is used temporarily for a period of time until a more definitive prosthesis can be provided .
Definition: is dental prosthesis to be used for a short interval of time for reasons of esthetics, mastication, occlusal support, or convenience.
objective of using an interim prosthesis is to condition the patient to the acceptance of an artificial substitute for missing natural teeth until more definitive prosthodontic therapy can .be provided

Interim Removable Partial Dentures

Transitional Denture
-Transition to a complete denture -Teeth need to be extracted but not immediately (medically compromised) -Patient is not psychologically prepared to loose all the teeth at one time

Transitional Denture

Treatment Denture Improve a condition before a definitive denture can be made
Treatment Denture may include: -Tissue conditioning papillary hyperplasia (massage, brushing, with or without surgery) - acute inflammation (increase tissue adaptation and redistribute the stress) - may use the existing denture or a new treatment denture may be made -Implant healing -Alteration of Vertical Dimension /Occlusion -Surgical Splint -Removal of palatal tori

Immediate RPD

Any removable dental prosthesis fabricated for placement immediately following the removal of a natural tooth/teeth. It is a partial denture constructed before the extraction of unwanted teeth and is inserted immediately after their removal. An immediate denture, which may be complete or partial, is constructed to replace teeth immediately after their extraction.

Indications of Temporary Removable Partial Dentures

1. Esthetic or appearance. 2. Space Maintenance . 3. Reestablishment of occlusal relationships 4. To condition teeth and residual ridges 5. Interim restoration during treatment 6. To condition the patient for wearing a prosthesis

1. Esthetic or appearance. For the sake of appearance, a temporary removable partial denture may replace one or more missing anterior teeth or it may replace several teeth, both anterior and posterior

2. Space Maintenance In younger patients the space should be maintained until the adjacent teeth have reached sufficient maturity to be used as abutments for fixed restorations In adult patients the maintenance of the space can prevent undesirable migration and extrusion of adjacent or opposing teeth until definitive treatment can be accomplished.

3. Reestablishment of occlusal relationships

Temporary removable partial dentures may be used as occlusal splint Temporary removable partial dentures are used for the following reasons: (1 ) to establish a new occlusal relationship or occlusal vertical dimension and (2) to condition teeth and ridge tissue for optimum support of the definitive removable partial denture that will follow.

4. To condition teeth and residual ridges

stimulation is from occlusal or finger pressure, there seems to be little doubt that the tissue of the residual ridge becomes more capable of supporting a distal extension removable partial denture when they have been previously conditioned by wearing a restoration.

5. Interim restoration during treatment Sometimes a temporary removable partial denture must be made to replace missing anterior teeth in a partially edentulous arch, which are ultimately to be replaced with fixed restorations
6. To condition the patient for wearing a prosthesis A temporary restoration may be made to aid the patient in making a transition to complete dentures

Advantages of acrylic partial dentures over Cobalt Chrome partial dentures

Light in weight. Good appearance Not expensive (Low cost). Easy to construct and to repair. Less laboratory and clinical time consuming
Disadvantages of acrylic partial denture
Poor thermal conductivity Lower strength (easily broken) Less hygienic Tendency for warpage if overheated during polishing.

Fabrication
Clinical Steps for Interim Removable Partial Dentures 1. History, examination, and proper treatment planning 2. Mouth preparation (endodontic, periodontal, surgery) 3. Upper and Lower impressions 4. pour the casts 5. Maxillo-mandibular relations (occlusal vertical dimension, centric record ) 6. Articulate the casts 7. Draw design for interim partial denture

Interim Partial Denture Design: Clasps (Wrought wire)

Design
Bracing Lingual/palatal major connector provides bracing Contacts teeth at the heights of contour
Major Connectors Full palatal coverage increases strength & stability Extend denture to first molar Retentive clasps embedded into major connector

ADDITION OF TOOTH TO THE DENTURE

Utilizing Acrylic Interim Partials
1. Generally, patients who wear interim partials can be more susceptible to caries as the acrylic interim partial denture (as well as remaining natural teeth) can become target for plaque accumulation. Therefore, extra prophylactic measures such as more frequent hygiene visits and regular use of fluoride should be recommended.

2. Ideally, interim partials should not be worn during sleep, as soft tissue needs time to recover to its normal healthy architecture. Patients who insist on wearing their prosthesis while they sleep should leave their partial out for several hours during the day.3. When using clasps for retention, care should be taken not to interfere with patient’s normal occlusion. Occlusal interferences are one of the main reasons for poor patient compliance with these appliances.

Thank You!
